Advance Health Care Directives

An Advance Health Care Directive, sometimes referred to as a “Durable Power of Attorney for Health Care Decisions” or a “Living Will”, allows you to appoint another individual as your health care agent to make healthcare decisions for you when you can no longer make them yourself. In this document, you may also state your wishes regarding matters such as life support
If you are unable to make healthcare decisions for yourself and do not have an Advance Health Care Directive, then a Conservatorship proceeding must take place for the court appointment of a conservator to make decisions on your behalf.

Having a properly executed Advance Health Care Directive will ensure continued management of your health care if you become incapacitated while avoiding unnecessary court involvement and excessive costs and fees.
